Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a remote, work-from-home capacity. This seasonal assignment runs through June 2027, with potential bonuses based on performance and business needs.

Responsibilities include telephonic risk assessments, care planning, education, and coordination of services to improve member outcomes. Collaboration with care teams and providers is essential.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently in a remote environment with nursing judgment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple EMR systems and electronic records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized, measurable care plans based on needs and risks.
• Establish health coaching relationships and provide education and support.
• Assess needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning with resources.
• Help navigate benefits, services and care options.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams to optimize care.
• Promote member self-management with education and resources.
• Document interactions, assessments and outcomes accurately in systems.
