Sagility is seeking a Senior Care Specialist (Registered Nurse) to perform remote telephonic health assessments and develop individualized care plans for Medicare and Medicaid members. This seasonal, work-from-home role emphasizes coordination of care, patient education, and collaboration with multidisciplinary teams.

Ideal candidates hold an active RN license, a Compact RN license, and 2+ years of clinical nursing experience, with strong judgment and communication skills.

Qualifikationen

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum 2 years clinical nursing experience (3+ preferred).
• Strong clinical judgment, critical-thinking, and problem-solving.
• Ability to work independently in a remote environment.
• Proficiency with Word, Excel, and Outlook; able to navigate multiple EMR systems.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education, encouragement and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
