Job summary

Sagility is seeking an experienced Registered Nurse for a Senior Care Specialist role to work from home in the United States for a seasonal assignment ending in June 2027. The RN will conduct telephonic health assessments and develop personalized care plans for Medicare/Medicaid members, coordinating with multidisciplinary teams and supporting transitions of care.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency with Word, Excel and Outlook.
• Ability to navigate multiple computer systems and EMRs.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate benefits, services, and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote self-management through education and resources.
• Coordinate care activities among members, caregivers, providers.
• Document interactions, assessments, and outcomes timely in systems.
