Job summary

Sagility is seeking an experienced Registered Nurse for a Senior Care Specialist role that is work-from-home. This seasonal opportunity involves telephonic health assessments, care plan development, and coaching to support chronic conditions for members in Medicare/Medicaid or group plans.

Candidates must hold an active RN license and a compact license where applicable, with at least 2 years of clinical nursing experience. Remote work and strong communication are essential.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME, home health services, and appointments.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being in home environments.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, and providers.
• Promote member self-management with education and resources.
• Coordinate care and condition-management activities among members, caregivers, providers.
• Document interactions, assessments, care plans, interventions, and outcomes accurately.
