Job summary

Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. This seasonal role focuses on telephonic health assessments for Medicare, Medicaid, and group health plan members, and on developing personalized, measurable care plans.

You will coordinate transitions of care, educate members, and collaborate with a multidisciplinary team to support chronic condition management. Remote, flexible work with strong nursing judgment required.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, and provider appointments.
• Help members and caregivers navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors that may affect a member's health and overall well-being.
• Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote member self-management by providing education and resources appropriate to individual health conditions and goals.
• Coordinate care and condition-management activities among members, caregivers, providers, and other members of the care team.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately and in a timely manner within applicable systems.
