Job summary

SCAN Health Plan is seeking a Sr. Care Coordinator (Behavioral Health) for a remote role based in Long Beach, CA. The position supports age-in-place care by coordinating behavioral health and medical services for SCAN members.

You will screen and refer, collaborate with care teams, and connect members to therapy, psychiatry, and social supports. Strong communication and motivational interviewing help de-escalate issues and improve engagement.

Qualifications

• High School Diploma required.
• Some college preferred.
• 5+ years in managed care, healthcare environment, or behavioral health case management.
• 2+ years working with seniors and remote work strongly preferred.
• Bilingual Spanish strongly preferred (conversational, reading, writing).

Responsibilities

• Contribute to the care and well-being of SCAN members by assessing, planning, coordinating, and monitoring holistic needs including behavioral health.
• Provide guidance to reduce barriers to care and improve engagement across services.
• Coordinate referrals, connect members to resources, and navigate the healthcare system with empathy.
• Communicate with members, providers, and care teams for coordinated care and timely access.
• Document interactions, progress, and outcomes in EMR and tracking databases.
