Job summary

SCAN Health Insurance is hiring for a remote Behavioral Health Care Coordinator role focused on helping seniors age in place. You will screen, refer, and coordinate care across behavioral health and medical services, while supporting navigation and addressing barriers with compassion.

You will use motivational interviewing to engage members, educate on care options, and work with internal/external teams to ensure timely access to services. Remote position with strong benefits.

Qualifications

• High School Diploma required; some college preferred.
• Minimum 5+ years in managed care, healthcare, or behavioral health case management.
• 2+ years working with seniors; remote work strongly preferred.
• Spanish bilingual skills preferred (conversational).
• Strong understanding of CMS and DHCS regulations.

Responsibilities

• Screen members for behavioral and medical needs and refer to appropriate services.
• Coordinate care, referrals, and benefits across teams and providers.
• Support navigation of the healthcare system and resolve escalated issues.
• Educate members on care journeys, telehealth, psychiatry, and therapy options.
• Document interactions and outcomes in EMR and tracking systems.
