Qualifications

• High School Diploma required.
• Bilingual English and Spanish or Vietnamese, or Cantonese or Mandarin; a test will be administered.
• 1+ year of experience in healthcare/customer service.
• Basic technical skills for the functional area.
• Problem solving and good interpersonal skills.
• Excellent written and verbal communication.

Responsibilities

• Assess and address member needs using holistic care planning.
• Coordinate referrals, services, and benefits access.
• Document assessments, care plans, and notes in multiple systems.
• Collaborate with providers, vendors, and county offices as needed.
• Maintain professionalism and confidentiality in all interactions.
