Job summary

SCAN Health Insurance is seeking a remote Behavioral Health Care Coordinator to support older adults in navigating complex care needs. The role emphasizes assessment, care planning, and referrals to behavioral health and medical services, with a focus on member engagement and compassionate coordination.

You will work across internal and external teams, use motivational interviewing techniques, and support access to care while documenting in EMR systems.

Qualifications

• High School Diploma required.
• Some college preferred.
• 5+ years working in managed care, healthcare environment, or behavioral health case management.
• 2+ years working with seniors and remotely (preferred).
• Strong communication and interpersonal skills; bilingual Spanish preferred.

Responsibilities

• Contribute to the care and well-being of SCAN members by assessing, planning, coordinating, and monitoring holistic needs including behavioral health and psychosocial aspects.
• Provide guidance and care coordination to reduce barriers to care and improve engagement across behavioral health and medical services.
• Focus on care coordination needs, processing referrals, connecting to resources, and navigating the healthcare system; resolve escalated issues with compassion.
• Perform general and behavioral health screenings and provide timely referrals.
• Participate in special projects and act as SCAN liaison with behavioral health providers.
• Coordinate care using member benefits and collaborate with internal/external teams to improve access to care.
