Job summary

Oak Street Health is hiring a Clinical Call Center Triage Nurse for remote work. You will extend care by addressing patients' medical, social, and psychological needs telephonically, across OSH practice states.

A valid RN license and ability to work flexible hours are required. Responsibilities include handling prescriptions, documenting meticulously, coordinating appointments, and arranging patient transportation.

Qualifications

• US work authorization.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Engage with patients with focus and minimal distractions.
• Document clearly and precisely during calls.
• Complete all patient requests documentation before ending a call.
• Schedule appointments for off-cadence requests or when needed.
• Arrange transportation for clinic visits.
