Oak Street Health is seeking a Clinical Call Center Triage Nurse for a remote role. The nurse extends care telephonically, addressing medical, social, and psychological needs and supporting OSH patients beyond clinic hours.

The position requires an active RN license, reliability, and the ability to work flexible shifts, with a comprehensive benefits package and ongoing remote training.

Qualifications

• Active, non‑probationary RN license in all states of OSH practice.
• Ability to manage time and work autonomously with consistent reliability.
• High level of conscientiousness and focus on patient experience.
• Willingness to work flexible shifts outside core hours, including evenings, weekends, and holidays.
• US work authorization.

Responsabilités

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly while on a call and complete requests prior to ending a call.
• Schedule appointments and arrange transportation for clinic visits as needed.
• Maintain alignment with departmental metrics and participate in remote training.
