Responsibilities

• Provide confident and professional service to Oak Street Health patients.
• Follow current Prescription Refill policy.
• Remain engaged with patients without background distractions.
• Clear and precise documentation while on calls.
• Complete documentation for all patient requests prior to ending a call.
• Check CareReq queue and ensure completion of CareReqs.
• Schedule appointments and arrange transportation as needed.
• Contribute to meeting departmental metrics and goals.
• Perform other duties as assigned.
