Job summary

Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care by addressing patients' medical, social, and psychological needs over the phone. The role supports clinic hours and may require remote work, with a focus on accurate, compassionate patient interactions.

You will manage documentation, coordinate appointment scheduling and transportation, and work toward performance metrics while maintaining licensure in all states OSH practices.

Qualifications

• Active RN license in all OSH practice states.
• Experience in patient-facing roles is preferred.
• Ability to work remotely with a distraction-free environment.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly and precisely during and after calls.
• Complete all patient requests prior to ending a call.
• Schedule appointments for patients as needed.
• Arrange transportation for clinic visits.
• Maintain alignment with departmental metrics.
• Handle other duties as assigned.
