Sentara Health in Roanoke, VA seeks an Integrated Case Manager, RN to perform remote case management with in-person assessments. Travel to Roanoke and northern areas is required up to 60-90 minutes for member visits, roughly 50–75% of the week.

The role involves coordinating and evaluating member care plans across physical and behavioral health and social services. Requires RN licensure, associates degree (bachelor's preferred), and at least 3 years of nursing experience.

Qualifications

• RN license required (Registered Nurse).
• Minimum 3 years of nursing experience.
• Managed care experience preferred; discharge planning experience preferred.

Responsabilités

• Provide case management within licensure scope and develop care plans.
• Perform telephonic assessments to coordinate health and social needs.
• Coordinate authorizations/referrals within benefits structure.
• Interface with Medical Directors and interdisciplinary teams on care plans.
• Identify high-risk members and ensure care across the continuum.
• Support regulatory compliance and problem-solve with providers.
