Sentara Health is seeking an Integrated Case Manager, RN to provide remote case management with in-person assessments in Roanoke, VA. The role requires travel to Roanoke and northern areas up to 60-90 minutes per week, with telephonic and in-person care coordination to optimize member health across the continuum.

Requires RN license, Associates degree (Bachelors preferred), and 3 years of nursing experience; managed care and discharge planning experience are preferred.

Qualifications

• Associates degree required; Bachelors preferred.
• Registered Nurse license required.
• Minimum 3 years of nursing experience.
• Managed care and discharge planning experience preferred.
• Willingness to travel up to 60-90 minutes.

Responsibilities

• Perform case management within licensure; develop and revise care plans.
• Conduct telephonic clinical assessments for care coordination.
• Identify high-risk members and coordinate with care team.
• Facilitate authorizations/referrals within benefits.
• Present cases at multidisciplinary case conferences.
• Ensure compliance with regulatory and company policies.
