Job summary

Sentara Health is hiring an Integrated Case Manager, RN. The position is remote but requires in-person face-to-face assessments, with travel to Roanoke and northern areas up to 60-90 minutes, roughly 50-75% of the week.

The role involves case management within licensure scope, developing and revising care plans, coordinated telephonic assessments, and collaboration with health care teams to optimize member health across the continuum.

Qualifications

• 3 years of nursing experience required.
• Registered Nurse license required.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Provide case management across the care continuum and develop care plans.
• Perform telephonic clinical assessments for member needs.
• Coordinate and monitor physical/behavioral health, social services, and long-term care.
• Interface with medical directors and care teams on treatment plans.
