Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This position is remote but does require in-person face-to-face assessments. Travel to Roanoke and northern areas is required up to 60-90 minutes, approximately 50 to 75% of a week.

The Integrated Case Manager manages casework across physical, behavioral health, social services and long-term care, developing and revising care plans to optimize health outcomes. Strong communication, problem-solving, and analytical skills are essential.

Qualifications

• Active RN license required.
• 3 years nursing experience required.
• Managed care preferred.
• Discharge planning experience preferred.

Responsibilities

• Perform case management within licensure scope; develop, monitor, and revise member care plans.
• Conduct telephonic clinical assessments for identification, evaluation, and management of needs across physical/behavioral health, social services, and long-term services.
• Identify high-risk members and coordinate care with health care team; manage chronic illnesses and disabilities with cost-effective utilization of benefits.
• Facilitate authorizations/referrals and interface with Medical Directors/Physician Advisors and interdisciplinary teams.
