Job summary

Sentara Health is seeking an Integrated Case Manager, RN, to perform case management across physical, behavioral health, and social services. This role is remote but requires in-person face-to-face assessments and travel to Roanoke and northern areas up to 60–90 minutes, 50–75% of the week.

The candidate will develop and monitor care plans, coordinate care with the health team, and ensure cost-effective utilization of benefits.

Qualifications

• RN license required
• 3 years nursing experience
• Managed care preferred
• Discharge planning experience preferred

Responsibilities

• Perform case management telephonically and in person to coordinate and optimize member care.
• Develop, monitor, evaluate, and revise member care plans.
• Coordinate authorizations/referrals within benefits structure and ensure regulatory compliance.
