Sentara Health is seeking an Integrated Case Manager, RN. This remote role requires in-person face‑to‑face assessments and travel to Roanoke and northern areas up to 60–90 minutes several times a week.

The role focuses on developing and evaluating member care plans, performing telephonic assessments, coordinating care across physical and behavioral health, social services and long-term supports. Strong emphasis on case management within regulatory and company policies.

Competenze

• Must be an RN with integrated case management experience.
• Bachelor’s degree preferred.
• Managed care experience preferred.
• Must be able to travel 60–90 minutes for member visits.

Mansioni

• Develops, monitors, and revises the member's care plan across the care continuum.
• Performs telephonic clinical assessments for needs identification, evaluation and coordination of physical, behavioral health, social services and long-term supports.
• Identifies high-risk members and coordinates care with the care team.
• Facilitates authorizations/referrals within benefits structure or extras.
• Ensures compliance with regulatory, accrediting, and company policies and procedures.
• Interfaces with Medical Directors and Inter-Disciplinary Teams on care plans.
