Job summary

Sentara Health is seeking an Integrated Case Manager, RN, to deliver comprehensive case management across the care continuum. This remote position requires in-person face-to-face assessments and travel to Roanoke and surrounding areas up to 60–90 minutes, approximately 50–75% of each week.

You will develop and monitor care plans, perform telephonic assessments, and coordinate with Medical Directors and care teams.

Qualifications

• Bachelor's degree preferred.
• Managed care experience preferred.
• Discharge planning experience preferred.
• Ability to perform telephonic clinical assessments.

Responsibilities

• Perform telephonic clinical assessments to identify, evaluate, and coordinate members' needs across physical, behavioral health, social services and long-term care.
• Develop, monitor, evaluate, and revise member care plans to optimize health care across the continuum.
• Identify high-risk members and coordinate with the health care team and Medical Directors/Physician Advisors.
• Assist with authorizations/referrals within benefits structures or extra-contractual arrangements.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
