Sentara Health is seeking an Integrated Case Manager, RN. This remote position requires in-person face-to-face assessments with travel to Roanoke and northern areas up to 60-90 minutes, approximately 50–75% of the week.

The role focuses on developing, monitoring, and revising care plans to optimize member health across the care continuum. You will perform telephonic clinical assessments, coordinate care with the health team, and ensure compliance with regulations.

Competenze

• RN licensure required or preferred
• Bachelor's degree preferred
• Experience in managed care and discharge planning is valued
• Strong communication and collaboration skills

Mansioni

• Perform telephonic clinical assessments to identify, evaluate, and coordinate member needs across physical, behavioral health, social services and long-term care.
• Develop, monitor, evaluate and revise the member care plan to optimize health across the care continuum.
• Identify high-risk members and coordinate care with the health care team to manage chronic illnesses and comorbidities.
• Facilitate authorizations/referrals within benefits structure or extra-contractual arrangements as permissible.
• Interface with Medical Directors, Physician Advisors and Inter-Disciplinary Teams on care plans and ensure regulatory compliance.
