Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This position is remote but requires in-person face-to-face assessments.

Candidates must be able to travel to Roanoke and northern areas as travel is required up to 60-90 minutes to see members approximately 50 to 75% a week. The Integrated Case Manager develops, monitors, evaluates and revises the member's care plan, performs telephonic clinical assessments, and coordinates care across physical and behavioral health, social services and

Qualifications

• RN licensure required or eligible.
• Bachelor's degree preferred.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Perform telephonic clinical assessments for identification, evaluation, coordination and management of member's needs.
• Develop, monitor, evaluate and revise member care plans to meet member needs across the care continuum.
• Coordinate care with Medical Directors and Inter-Disciplinary Teams on care management plans.
• Ensure compliance with regulatory, accrediting and company policies and procedures.
• Assist with authorizations/referrals within benefits structure or extra-contractual arrangements.
