Job summary

Sentara Health is hiring an Integrated Case Manager, RN. This role is remote but requires in-person assessments with travel to Roanoke and surrounding areas up to 60–90 minutes, roughly half of each work week.

The incumbent coordinates care across physical and behavioral health, social services, and long-term supports, ensuring optimal member outcomes. The position requires an RN license, minimum three years of nursing experience, and experience with managed care or discharge planning.

Qualifications

• Licensed RN with active license.
• Minimum 3 years nursing experience.
• Travel up to 60–90 minutes for member visits.
• Managed care experience preferred; discharge planning experience preferred.

Responsibilities

• Perform telephonic clinical assessments to identify, evaluate, coordinate, and manage member needs.
• Develop, monitor, evaluate, and revise individual care plans across the care continuum.
• Coordinate with medical directors, physician advisors, and interdisciplinary teams on care plans.
• Present cases at case conferences for multidisciplinary review; ensure compliance with policies and procedures.
• Assist with problem solving with provider, claims, or service issues.
