Job summary

Enlyte is seeking a Field Case Manager for a full-time, work-from-home role with regular local travel for in-person patient appointments in the San Antonio, TX area. You will partner with physicians, employers, and patients to create and implement treatment plans that restore function and support safe, cost-effective recovery.

The ideal RN has 2+ years of clinical experience, an active RN license, and ideally URAC case management certification. Bilingual English/Spanish is preferred.

Qualifications

• RN license active and in good standing.
• 2+ years of clinical practice preferred.
• URAC-recognized case management certification preferred.

Responsibilities

• Develop and implement treatment plans with physicians and patients.
• Coordinate with employers to modify job duties for safe return to work.
• Attend in-person appointments as required and manage caseloads.
