Enlyte is seeking a Field Case Manager (RN) in the Oceanside, CA area for a full-time hybrid role. You will collaborate with physicians, employers, and patients to create effective, cost-conscious treatment plans that return workers to productive activity. Regular travel to in-person appointments is required.

Qualifications

• RN license active and in good standing.
• URAC-recognized case management certifications (CCM/CDMS/CRC/CRRN/COHN etc.).
• Able to advocate recommendations with stakeholders and work independently.
• Strong computer skills: Excel, Word, Outlook; excellent written communication.

Responsibilities

• Apply case management standards and provide treatment plan guidance.
• Assess recovery progress and occupational vs non-occupational factors.
• Coordinate with physicians, employers, and patients to adjust plans.
• Facilitate early return-to-work by modifying job duties where possible.
• Attend in-person appointments and prepare status updates for clients.
