Job summary

Enlyte is seeking a Field Case Manager to support in the San Antonio area. This full-time, work-from-home role requires occasional in-person visits for injured workers within your geography. You will collaborate with physicians, employers, and caregivers to create treatment plans, advocate for cost-effective care, and coordinate timely return-to-work strategies.

Bilingual English/Spanish is preferred. Compensation ranges from $71,000 to $80,000 plus bonus; comprehensive benefits and home-office

Qualifications

• Associates Degree or Bachelor’s Degree in Nursing or related field.
• RN license in good standing.
• URAC-recognized case management certifications preferred.

Responsibilities

• Create and implement treatment plans tailored to an individual patient.
• Collaborate with treating physicians/providers and employers to tailor care.
• Advise on modification of job duties to facilitate early return to work.
• Independently manage workload and prioritize cases effectively.
• Attend injured worker appointments and prepare status updates for customers.
