Job summary

Enlyte is seeking a Field Case Manager to work from home with regular travel to Salt Lake City for in-person patient appointments. You will coordinate with physicians, employers, and injured workers to create and implement effective treatment plans aimed at safe, cost-conscious recovery and return to work.

Qualifications

• Education: Associates Degree or Bachelor’s Degree in Nursing or related field.
• Experience: 2+ years clinical practice preferred.
• Skills: Advocacy with physicians/providers, employers, and customers; ability to work independently; basic Excel/Word/Outlook.

Responsibilities

• Demonstrate knowledge of case management standards of practice.
• Advise on treatment plans using medical knowledge and job duties.
• Interview injured workers to assess recovery.
• Collaborate with physicians to tailor treatment plans.
• Modify job duties to enable early return to work.
• Manage workload and prioritize cases.
• Attend in-person appointments when needed; prepare status updates.
