Job summary

CVS Health is seeking an RN Field Assessor to travel up to 85% across Morris County and surrounding NJ counties. This role blends home-based work with in-person assessments to help members access care and services, including adult daycare, personal care, and facility-based support.

Qualifications

• Active, unrestricted RN license in New Jersey.
• 1+ years clinical experience (hospital, home health, or ambulatory care).
• Comfortable working with diverse populations in various settings.
• Strong attention to detail, organizational skills, and ability to work independently.
• Computer proficiency and experience handling medical records/EMR.

Responsibilities

• Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
• Help members access vital services such as Adult Medical Daycare, Personal Care, Nursing Facility Custodial Care, Personal Preference Program, and MLTSS Program Enrollment.
• Complete 10+ assessments per week using a holistic approach considering co-morbidities and functional limitations.
• Collaborate with team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover health needs.
• Mentor new team members after mastering the role.
• Operate at full capacity by month four with comprehensive training and support.
• Develop proactive care plans to address issues and improve short- and long-term outcomes.
• Assess need for referrals to clinical resources and interdisciplinary team members.
