Job summary

CVS Health is seeking a Case Manager Registered Nurse for field work in Union County, NJ, with up to 75% travel. The role features a Monday–Friday 8am–5pm EST schedule, focusing on care planning, coordination, and utilization of resources for MLTSS/D-SNP members.

The ideal candidate will have 3+ years of clinical practice, an active NJ RN license, and NJ Choice certification. You’ll work in a dynamic team to mentor new hires and ensure high-quality patient care.

Qualifications

• 3+ years clinical practice experience in hospital or equivalent
• Active and Unrestricted RN license in NJ
• Must reside in Union County, NJ

Responsibilities

• Conduct face-to-face home visits for MLTSS/ D-SNP members to assess medical needs
• Develop care plans and authorize services within budget
• Coordinate with PCP and care team, document in EHR
• Mentor new hires after proficiency is reached
• Attend interdisciplinary meetings and advocate for members
