Qualifications

Responsibilities

• Conduct face-to-face home visits using comprehensive assessments for MLTSS/D-SNP/FIDE members.
• Develop and implement care plans and authorize services cost-effectively.
• Coordinate with care team, PCP, and authorized representatives; document in EHR.
