Job summary
CVS Health is seeking a Case Manager Registered Nurse - Field to deliver comprehensive case management to members in Hudson County and surrounding counties. This role involves up to 75% field travel, home visits, and collaboration with a multidisciplinary team to promote wellness and appropriate service utilization.
The position requires 3+ years of clinical practice, an active NJ RN license, and successful completion of NJ Choice certification as a contingency.
Qualifications
- 3+ years clinical practice in hospital or equivalent setting.
- Active and Unrestricted RN license in NJ.
- Must reside in Hudson and/or Surrounding Counties, NJ.
- Willing to travel up to 75% in Hudson and surrounding counties; mileage reimbursed.
- NJ Choice certification completed or must be obtained as contingency of employment.
Responsibilities
- Conduct face-to-face home visits to assess medical needs for MLTS/D-SNP and related programs.
- Develop care plans and authorize services within MLTSS/ FIDE guidelines, cost-effectively.
- Coordinate with care team, PCP, and authorized representatives; mentor new hires.
- Document accurately in the member's EHR and participate in interdisciplinary meetings.
- Advocate for safe discharge and ensure appropriate services are in place.
