Job summary

CVS Health is seeking a Registered Nurse Case Manager – Field in New Jersey to coordinate care for members with complex needs, helping them live safely at home. This role involves in-home assessments, developing care plans, and interprofessional collaboration to ensure holistic care.

The position requires 3+ years of clinical nursing experience, an active NJ RN license, and NJ Choice Certification, with significant local travel and mentorship opportunities within a supportive team.

Qualifications

• Associate’s degree in Nursing required; BSN preferred.
• At least 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
• Active, unrestricted RN license in New Jersey required.
• NJ Choice Certification with minimum exam score of 80% and field mentoring component.

Responsibilities

• Conduct in-home assessments with members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to support appropriate care and services.
• Collaborate with providers, caregivers, and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans to promote safety and independence.
• Participate in care team meetings and advocate for member needs.
• Document all assessments and care activities accurately in the EHR.
• Support members through care transitions and connect them with community resources.
• Mentor new team members as you grow in the role.
