Qualifications

• Active and Unrestricted RN license in NJ.
• 3+ years clinical practice in hospital or similar setting.
• Reside in Hudson/ surrounding counties, NJ and travel up to 75%.

Responsibilities

• Conduct face-to-face home visits using assessment tools for MLTS/DSNP members.
• Develop care plans and authorize services in a cost-effective manner.
• Coordinate care with PCP and care team; document in EHR.
• Attend interdisciplinary meetings and mentor new hires after proficiency.
• Communicate effectively to ensure safe discharge and appropriate services.
