Qualifications

• 3+ years clinical practice experience in hospital, home health, or ambulatory care.
• Active and Unrestricted RN license in NJ.
• Willing to travel up to 75% in Bergen County and surrounding areas.

Responsibilities

• Conduct face-to-face home visits for MLTSS/D-SNP members.
• Develop care plans and authorize services cost‑effectively within benefits.
• Document accurately in the member's electronic health record.
