Job summary

St. Luke's University Health Network is seeking a Clinical Triage Specialist (RN) to join the Access Center. The role uses nursing judgment to triage patient calls, provide clinical guidance, and coordinate appropriate care paths, while upholding patient safety and service quality.

Graduate of an accredited nursing program with active RN licensure in PA/NJ (or compact state) is required. Minimum 2 years of clinical experience in physician offices, home health, critical care, or ED is expected.

Qualifications

• Graduate of an accredited nursing program.
• Active RN licensure in the state(s) PA/NJ or a nursing compact state.
• Minimum 2 years of recent clinical experience in a physician office, home health, critical care, or emergency room.

Responsibilities

• Answers telephones and triages calls following clinical protocols; coordinates services as needed.
• Verifies patient demographics and updates records in the EHR.
• Serves as escalation point for clinical issues and provides guidance per protocols.
• Manages escalated electronic patient messages and prioritizes based on clinical judgment.
• Strives to create a positive patient experience and resolves issues within scope of role.
• Meets productivity, schedule adherence, and quality standards in Access Center.
• Uses resources to assess, advise, schedule appointments, or refer to appropriate facility/personnel.
• Documents symptoms, nursing assessment, advice given, and patient response.
• Collaborates with Access Center teams and practice clinicians on patient concerns; handles meds refills or scheduling as needed.
• Performs other duties as assigned.
