Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This remote role requires in‑person assessments and travel up to 60–90 minutes to sites in Roanoke and surrounding areas.

You will manage member care plans across the care continuum and coordinate telephonic assessments with the health care team. Responsibilities include coordinating authorizations/referrals, identifying high‑risk members, and ensuring compliance with policies.

Qualifications

• RN licensure required or equivalent
• Bachelor's degree preferred
• Managed care experience preferred

Responsibilities

• Perform telephonic clinical assessments to identify, evaluate and coordinate member needs.
• Develop, monitor and revise member care plans across the care continuum.
• Identify high‑risk members and coordinate care with the health team.
• Coordinate authorizations/referrals within benefit structures or arrangements.
• Interface with Medical Directors/Physician Advisors and Inter‑Disciplinary Teams to plan care.
• Ensure compliance with regulatory, accrediting and company policies.
