Qualifications

• 3+ years of experience in patient navigation, care coordination, or related roles.
• Experience with remote patient engagement and EHR documentation.

Responsibilities

• Build trusting relationships with patients and care teams.
• Navigate patients to medical, behavioral health, and community resources.
• Identify medical, behavioral health, social, and financial needs for whole-person care.
• Support patients in understanding care plans and follow-up steps.
• Document interactions and outcomes accurately in EHR systems.
