Qualifications

• 3+ years of patient navigation or care coordination experience.
• Experience with remote work and collaboration with clinical teams.
• Familiarity with EHR systems and documentation practices.

Responsibilities

• Build trusting relationships with patients, families, caregivers, and providers.
• Engage high-risk populations to address health questions and barriers.
• Identify and assess medical, behavioral health, social, and financial needs for whole-person care.
• Support patients in understanding care plans, goals, and follow-ups.
