Guidehealth is a data-powered, performance-driven healthcare company focused on affordable care and better patient outcomes. We seek Healthguides to strengthen connections between patients and their primary care teams, primarily working remotely to support care plans and navigation.

You will document encounters in EHRs, coordinate referrals, and collaborate with medical practices while meeting KPIs. A Certified Medical Assistant or EMT with 2+ years of experience is required, with paid training

Qualifications

• Certified or Registered Medical Assistant OR Certified EMT with 2+ years of experience.
• Passion for coordinating care for patients with complex needs.
• Willingness to learn motivational interviewing and behavioral change strategies.
• Strong empathy, professionalism, and respect in all interactions.

Responsibilities

• Support patients remotely by coordinating with primary care teams to close care gaps.
• Document medical history and issues accurately in the EHR.
• Assist with referrals to specialty care and community-based resources.
• Navigate patients through referrals and prior authorizations.
• Communicate effectively via calls, texts, portals, and email.
• Meet assigned KPIs to improve patient health outcomes and reduce spend.
• Travel 10–15% to clinical sites for patient engagement and retinal exams with paid training.
