Job summary

Guidehealth is seeking a Healthguide to coordinate care with clinical teams and the patient’s primary care practice. You will help patients navigate care, solve issues, and improve health outcomes while maintaining strong documentation in the EHR.

This role emphasizes remote collaboration, compassionate patient engagement, and travel up to 10–15% to clinical sites near Atlanta as needed, with base pay between $21 and $24 per hour.

Qualifications

• Certified Medical Assistant or Certified EMT with 2+ years of experience.
• Love for coordinating care for patients with complex needs.
• Willingness to learn motivational interviewing and behavioral change.
• Strong interpersonal skills, empathy, and professionalism.
• Proficient in documenting patient encounters and using EHR systems.
• Knowledge of medical terminology and primary care workflows.
• Comfort with multiple tech platforms and Microsoft Office.

Responsibilities

• Coordinate care with patients, medical practices, and Guidehealth Case Management.
• Independently support patients while collaborating with the medical team.
• Address questions, connect patients to practices, and aid in high-impact care plans.
• Document patient history and issues accurately in the EHR.
• Navigate referral and prior authorization processes as needed.
• Maintain secure handling of PHI in line with policies.
