Job summary

Guidehealth seeks a Healthguide to remotely support patients and their primary care teams, coordinating care, addressing questions, and guiding through referrals and care plans. You will document care in the EHR and collaborate with clinicians to improve health outcomes while reducing costs.

This role requires a Certified Medical Assistant or EMT with strong communication, empathy, and experience with EHRs (EPIC, Cerner, or eClinicalWorks). Occasional travel to Atlanta-area sites may be required.

Qualifications

• Certified or Registered Medical Assistant or EMT with 2+ years of experience.
• Ability to document patient interactions accurately in the EHR.
• Strong interpersonal and empathetic approach when engaging patients and families.

Responsibilities

• Support patients remotely, coordinating with medical practice staff and Case Management.
• Document medical history and issues in EHR accurately.
• Navigate referrals and prior authorization processes.
• Engage patients via calls, texts, portals, and emails to schedule high-risk patients.
• Escalate medical issues to nurses or physicians as needed.
• Maintain confidentiality and security of PHI.
