Qualifications

• Minimum of 5 years as a Registered Nurse.

Responsibilities

• Create personalized care plans addressing social determinants and emotional well‑being.
• Coordinate care across providers and settings to ensure safety, quality, and continuity.
• Educate and empower members to navigate the healthcare system confidently.
