Imagine Pediatrics is a tech-enabled pediatric medical group focused on care for children with complex needs. We offer 24/7 virtual and in-home care, with a strong emphasis on collaboration with families, providers, and health plans.

The Patient Engagement Associate II will enroll and onboard children and caregivers into the program, requiring strong critical thinking, collaboration, and the ability to work autonomously in a startup environment.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with navigating social service agencies and resourcing for patient/family needs.
• CRM familiarity and strong documentation habits are a plus.

Responsabilités

• Research attributed patients to understand healthcare needs and barriers to enrollment.
• Outreach to caregivers by phone to enroll children and ensure seamless onboarding.
• Contact other health professionals (DME, home health, pharmacies, pediatricians) involved with attributed patients.
• Leverage relationships with providers to re-engage enrolled patients.
• Handle inbound calls from caregivers, plan service coordinators, and discharge planners.
