Job summary

Imagine Pediatrics is a tech-enabled, pediatrician-led medical group reimagining care for children with special health care needs. We offer 24/7 virtual first and in-home care, collaborating with families, providers, and health plans to remove barriers to quality care.

This remote role focuses on enrolling children and caregivers, performing research to tailor outreach, and coordinating with health professionals to ensure a seamless onboarding experience.

Qualifications

• 2+ years of healthcare experience in care coordination, community health, patient engagement, customer service, or sales.
• Proven success in a centralized contact center environment.
• Experience with resourcing within a patient or family facing role and navigating social service agencies.
• Strong documentation skills; prior CRM experience is a plus.
• Ability to collaborate in a tight-knit team and enroll caregivers effectively.
• Excellent listening and communication skills with diverse audiences.

Responsibilities

• Collaborate with engagement pod and enroll and onboard children and caregivers.
• Conduct research on attributed patients to understand needs and barriers to enrollment.
• Telephonic outreach to caregivers to enroll children and ensure seamless onboarding.
• Outreach to other healthcare professionals (DME, home health, pharmacies, pediatricians, etc.).
• Leverage relationships to support re-engagement of enrolled patients.
• Handle inbound calls from caregivers, health plan coordinators, and hospital discharge planners.
• Develop knowledge of SSI program and outreach to eligible caregivers.
• Coordinate embedded outreach with Provider Engagement Specialists when appropriate.
