Valeris is hiring a Care Coordinator to own patient cases from initiation to closure, including benefit investigations and prior authorizations. The role involves phone, email, and portal communication, adherence to SOPs, and collaboration with nurses to provide patient support.

You will use the Lynk system to document activities, determine program eligibility, and help patients access medications. Remote eligibility may be possible, with on-site options in Florida as needed.

Formación

• Detail oriented and patient-focused with strong organizational skills.
• Professional telephone etiquette and clear communication.
• Ability to multitask and manage changing priorities in a healthcare setting.
• Bi-lingual skills in Spanish and English are a plus.
• Customer service and healthcare industry experience is preferred but not required.

Responsabilidades

• Ownership of patient cases from initiation to closure including benefit investigations and prior authorizations.
• Assist patients and callers via email and calls using Lynk technology within SOPs.
• Coordinate nurse teaching and adherence services with field-based staff.
• Manage inbound calls and triage to appropriate resources.
• Document all activities in the Lynk system and maintain data accuracy.
