Job summary

Valeris in Indiana is seeking a Care Coordinator to manage patient cases from initiation to closure, performing benefit investigations, prior authorizations, and payer communications using Lynk technology.

The role emphasizes ownership, attention to detail, and patient focus, with duties including inbound/outbound calls, personalized case management, and coordination with nurse educators to support adherence and program enrollment.

Qualifications

• Bachelor’s degree strongly preferred or equivalent experience required.
• Customer service and healthcare industry experience is preferred but not required.
• Ability to work an 8.5-hour shift between 8 am to 9 pm, Monday to Friday, required.

Responsibilities

• Relationship management with patients, prescribers, and client stakeholders.
• Performs program welcome calls and post benefits investigations explaining coverage options.
• Manages inbound calls and triages patients to internal or external resources.
• Provides personalized case management including outreach to HCPs and patients.
