Valeris is seeking a Care Coordinator to own patient cases from initiation to closure, including benefit investigations, prior authorizations, and patient assistance coordination. You will use our Lynk system to document activities, communicate coverage options, and ensure high-quality patient support.

The role emphasizes ownership, attention to detail, and adherence to healthcare regulations, with remote work eligibility based on criteria and business needs.

Qualifications

Responsibilities

• Build trusted relationships with patients, prescribers, and client stakeholders.
• Perform program welcome calls and post-benefits investigation calls.
• Manage inbound calls and triage patients to appropriate resources.
• Provide personalized case management and communicate benefit coverage.
• Document activities in the PharmaCord Lynk system and ensure SOP adherence.
