Job summary

ComplexCare Solutions seeks a Nurse Practitioner per diem to conduct comprehensive home and telehealth assessments in Klamath County, OR. You will spend up to an hour per member to capture a complete view of needs, risks and care management opportunities aimed at keeping patients home and out of the hospital.

The role emphasizes CMS/HEDIS compliance, evidence-based practice, and timely documentation. Individual autonomy in a remote clinical setting is required, with ongoing communication to PCPs

Qualifications

• Active nursing license and board certification as NP (ANCC or AANP).
• Current CPR certification.
• Vaccination/testing compliance per healthcare policies.
• Ability to autonomously practice in a remote clinical environment.

Responsibilities

• Conduct comprehensive member assessments and document findings.
• Maintain CMS/DHHS and client documentation standards.
• Communicate results and care recommendations to PCPs and health plans.
• Identify and close gaps in care for chronic conditions.
• Perform Point of Care testing and document results.
• Uphold HIPAA and company policies.
