ComplexCare Solutions seeks a Nurse Practitioner per diem to perform in-home and telehealth assessments nationwide. Each member interaction aims to capture a comprehensive view of needs and care requirements to keep individuals safely at home.

The role requires an active NP license, board certification (Family/Adult/Gero/EM), CPR, and vaccination compliance. You will document findings, coordinate with PCPs/health plans, and help close care gaps while delivering high-quality care.

Qualifications

• Active un-encumbered nursing license.
• ANCC or AANP board certification as a Nurse Practitioner or Clinical Nurse Specialist in Family, Adult, Gerontology or Emergency Medicine.
• Maintains current CPR certification.
• Compliance with recommended vaccinations and preventive testing prior to hire: Hepatitis B, Influenza, MMR, Pertussis/Tetanus/Diphtheria (Tdap), Varicella, Tuberculosis.

Responsabilités

• Conduct comprehensive, accurate member assessments in home or telehealth settings.
• Document encounters and ensure CMS/DHHS compliance in medical records.
• Coordinate with physicians, PCPs and health plans; communicate findings and follow-up timelines.
• Identify and close gaps in care and provide evidence-based recommendations.
