Job summary

ComplexCare Solutions seeks a Nurse Practitioner for per diem home and telehealth assessments across Jackson County, OR. You will conduct comprehensive patient evaluations, build treatment plans, and collaborate with physicians and health plans to optimize care and outcomes.

The role emphasizes autonomous practice in a remote clinical setting, documenting findings, and ensuring regulatory compliance, including CMS/HEDIS guidelines and HIPAA.

Qualifications

• Active un-encumbered nursing license required.
• ANCC or AANP NP certification in Family, Adult, Gerontology or Emergency Medicine.
• CPR certification required.
• Compliance with vaccinations/testing (Hepatitis B, Flu, MMR, Tdap, Varicella, TB).
• Experience with CMS/HEDIS guidelines and HIPAA in home health settings.

Responsibilities

• Conduct comprehensive patient assessments during home visits.
• Develop evidence-based treatment plans and document findings.
• Coordinate with PCP and health plans for follow-ups and referrals.
• Ensure timely, accurate medical records and encounter documentation.
• Identify and close gaps in care and provide patient education.
